A new discharge summary for patients leaving hospital has been introduced in Gippsland.
Gippsland Primary Health Network (PHN) and the Gippsland Health Alliance launched the new format in November following 12 months of workshops and engagements.
The Electronic Medical Record (EMR) patient discharge summaries aim to alleviate doctor concerns with the previous format.
